To modify National Identification Number data, individuals must submit supporting documents and complete the required forms at a designated National Identity Management Commission office.

5. Proof of citizenship or residency (any of: birth certificate, passport, naturalization certificate, etc.)
6. Recent passport-sized photograph
7. Fingerprints (captured at the NIMC office)
8. Signature (signed on the modification form)
9. Payment of the applicable modification fee (if required)
10. Completed and signed consent form (for minors or individuals with legal guardians)
